    Planned Absence Form (posted soon)- Please fill out and give to the office at least 3 DAYS IN ADVANCE of a planned absence, more notice is better. 

    Authorization to Administer Medications at School - must be filled out and signed by parent AND doctor before ANY medications can be administered to students, including cough drops or other over-the-counter medicines.

    Volunteer Application - emergency contact information needs to be updated each school year you wish to volunteer
    Washington State Patrol Background Check - needs to be renewed each school year you wish to volunteer
    Volunteer Handbook Signature Page - please read the complete handbook on the volunteer page at Seattle Public Schools Volunteering and sign and return the Signature  page to the school.
    Volunteer Driver Checklist - must be filled out each year if you plan to drive for a field trip

    504 Form - Fill out and return to the office if you believe your child is disabled and his/her needs are not being met.

    Title VII Eligibility form - Fill out and return to the office if your student has Native American Heritage.  Only needed once while your student is at North Beach.

    New Seals Bulletin Board Permission Slip - We like to post photos of the students who are new to North Beach (and not in Kindergarten) to help other students and staff recognize them and learn their names. If your student is not pictured, please let us know.
